I/We understand and agree that the Personal Information will be collected and used by NewCo for the purpose of underwriting and to assess my suitability to act as a guarantor for revenue-based financing, loans and/or other financial instruments as offered by NewCo and for which I, or an entity, on behalf of which or for which I am a guarantor, may apply.
I/We further authorize and consent to the disclosure of my Personal Information as needed in order to underwrite the deal for the business entity that I/We represent or to assess my suitability as a guarantor, and agree that the collection, use and disclosure of my Personal Information is necessary for such purposes. I/We understand and agree that the Personal Information collected will be retained for as long as reasonably necessary for such purposes, and will be treated as otherwise contemplated by NewCo’s Privacy Policy. I/We agree that this consent is valid as of the date provided below, and shall persist for the duration of my agreement with NewCo.
